Many insurance companies require that you pre-certify for certain procedures. Check your policy or contact your agent to ensure that both physician and hospital services are properly authorized before proceeding.

After you have submitted this form, a financial counselor will be able to give you an estimate of your financial responsibility. Please call 601-968-1344 or 1-888-598-2405 to speak with a financial counselor.

If you do not have insurance coverage, please contact our Business Office at 601-968-1344 to make arrangements for payment.

To complete your admissions process, upon your arrival at Baptist you will need to:

• (1) provide photo ID for positive identification
• (2) provide copies of your insurance card(s) (Remember to bring your insurance card(s)
  with you each time you come for services at Baptist ); and
• (3) sign authorization forms.
